
Report Eye Injuries: May 11 to 18, 2008
Part of the EyeSmart Campaign
The Eye Injury Snapshot is an annual study conducted by the American Academy of Ophthalmology and the American Society of Ocular Trauma to identify the nature and prevalence of eye injuries.
How to Submit Reports
Filling out the form only takes a few minutes. Choose one:
All submissions are confidential and compliant with HIPAA guidelines. Reports must be received by Friday, May 23, to be included in this year's data.
Submitting Photos
Photos are not required as part of the report submission, but if you would like to provide an image, send it by e-mail to eyesmart@aao.org. They may be used in an article in EyeNet magazine or another publication. Digital images should be at least 266 dpi and saved as jpeg-, tif- or eps-files. We will need your patient's permission; use this photo release form (PDF 27K) to obtain patient consent.
Why Participate?
Eye injuries are the focus of the EyeSmartTM Campaign in 2008. The snapshot data will be combined with results from a new public survey (gauging consumer awareness on eye injuries and safety) to officially launch the awareness effort in June. Pairing the physician reporting and public survey will help increase media and consumer attention to this important issue. In addition, it will help reinforce EyeSmart's message about unique role ophthalmologists play in eye health.
Promoting the Eye Injury Snapshot
